Why does my porcupine puffer's gill look like it has closed up?

My puffer has not been eating for almost a week now and we weren't sure what wrong. I was looking at it real closely and I realized his one gill is closed and he is not able to breath on that side. Is there anything we can do? What caused this? WIll he be okay? Please someone let me know.
